ABSTRACT

Objective:To investigate the effect and mechanism of astragaloside IV (AS-IV) combined with glucocorticoids in the treatment of puromycin aminonucleoside (PAN) rat nephropathy model.Methods:Forty specific pathogen-free healthy male Wistar rats (150-180 g) were randomly divided into 5 groups: control group, PAN group, AS-IV treatment group (PAN+AS-IV group), methylprednisone (MP) treatment group (PAN+MP group), and AS-IV+MP treatment group (PAN+AS-IV+MP group). The model was established by a single tail vein injection of PAN (50 mg/kg body weight). The treatment groups were given 40 mg·kg -1·d -1 AS-IV by intragastric administration and 15 mg·kg -1·d -1 MP by intraperitoneal injection for 10 consecutive days at the same time of modeling. Urine sample was collected on the 11th day of the experiment. The urine protein, urine creatinine and blood albumin were detected by biochemical analyzer. The changes of nephrin and synaptopodin in renal tissues were detected by immunofluorescence assay, and the expressions of nephrin, RhoA and Rac/Cdc42 proteins were detected by Western blotting. Results:Compared with the control group, urine protein creatinine ratio (uPCR) was significantly increased, serum albumin (Alb) was significantly decreased in the PAN group, nephrin expression was significantly down-regulated, and the expressions of RhoA and Rac/Cdc42 were significantly up-regulated in the renal tissue of the PAN group (all P<0.01). Compared with PAN group, serum Alb levels in PAN+AS-IV group and PAN+AS-IV+MP group were significantly increased (both P<0.01), and the uPCR levels in PAN+MP group ( P<0.05) and PAN+AS-IV+MP group ( P<0.01) were significantly decreased (all P<0.05). Compared with the PAN group, the relative expressions of nephrin in renal tissue of all drug intervention group (PAN+AS-IV group, PAN+MP group and PAN+AS-IV+MP group) were significantly increased, while the relative expressions of RhoA and Rac/Cdc42 were significantly decreased (all P<0.01). The immunofluorescence results suggested that the expressions of nephrin and synaptopodin in renal tissue of PAN group were significantly down-regulated compared with the control group, which were reversed in all treatment groups, and the reversion was most pronounced in the PAN+AS-IV+MP group. Conclusion:Both AS-IV and glucocorticoid can improve PAN-induced podocyte injury, and the combination of the two has synergistic action, which may be related to inhibiting the activation of Rho family signaling pathway.

ABSTRACT

With in-depth studies on the etiology and pathogenesis, pathological viscera and bowels, and rule of disease progression of primary liver cancer in recent years, some clinical and experimental results have been achieved for the treatment of primary liver cancer with traditional Chinese medicine, such as treatment based on syndrome differentiation and disease differentiation, compound prescriptions and monomers of Chinese herbs, oral administration of drugs/acupuncture and moxibustion/external application, and prevention before disease onset and prevention of recurrence, which reflects the rich methodological connotations of traditional Chinese medicine and lays a foundation for constructing a new diagnostic and therapeutic mode for liver cancer based on integrated traditional Chinese and Western medicine therapy.

ABSTRACT

Primary liver cancer is one of the most common malignant tumors in China, and most patients are in the advanced stage at the time of diagnosis, with a short survival time and poor prognosis. In recent years, traditional Chinese medicine has shown its unique advantages in the prevention and treatment of primary liver cancer and has achieved a marked clinical effect in improving quality of life, enhancing immunity, and prolonging the life of patients with liver cancer. This article reviews the current status of traditional Chinese medicine and Western medicine in the treatment of primary liver cancer and proposes the breakthrough points and difficulties in traditional Chinese medicine treatment based on the features of clinical treatment, in order to provide a reference for the clinical treatment of primary liver cancer.

ABSTRACT

Treatment regimens for primary liver cancer mainly include surgical resection, liver transplantation, local treatment, and systemic treatment, but there are still problems such as easy metastasis, easy recurrence, and poor prognosis. Most patients are already in the advanced stage at the time of diagnosis and thus lose the opportunity for surgery and require multidisciplinary combined treatment. Traditional Chinese medicine (TCM) has unique advantages in the treatment of primary liver cancer and can exert a synergistic effect in collaboration with different Western medicine treatment methods. This article establishes TCM treatment regimens based on the different clinical stages of primary liver cancer, i.e., surgical resection for early-stage liver cancer to strengthen vital Qi and prevent recurrence, minimally invasive treatment of middle-stage liver cancer to eliminate cancer toxicity and improve efficacy, and symptomatic treatment of advanced liver cancer to reduce pain and maintain quality of life.

Liver cancer is one of the most common malignant tumors at present and has high incidence and mortality rates. Most patients are in the advanced stage at the time of diagnosis and thus lose the opportunity for surgery and have poor prognosis. At this time, medical treatment focusing on drug therapy becomes an important method for the treatment of advanced liver cancer, including chemotherapy, targeted therapy, immunotherapy, traditional Chinese medicine treatment, and endocrine therapy. This article reviews the recent research advances in drug therapy for advanced liver cancer.

ABSTRACT

OBJECTIVE@#To explore the clinical efficacy of exercise acupuncture and osteopathy in the treatment of traumatic knee arthritis.@*METHODS@#Fifty-one patients with traumatic knee arthritis were divided into three groups:routine group, exercise acupuncture group and osteopathy group. In the routine group, there were 17 cases, 12 males and 5 females, with a mean age of (49.4±8.9) years old; the averaged course of disease was (4.89±1.52) years; total WOMAC score before treatment was 100.77±15.48. Seventeen patients (11 males and 6 females) in the exercise acupuncture group were aged (48.6±10.1) years old; the course of disease was (4.21±1.37) years; and the total WOMAC score before treatment was 106.16±14.95. In the osteopathy group, there were 17 cases, 8 males and 9 females, with a mean age of (52.3±8.4) years old; the average course of disease was (4.79±1.50) years; total WOMAC score before treatment was 103.87±10.14. The patients in the routine group were treated with routine rehabilitation; the patients in the exercise acupuncture group were treated with routine rehabilitation combined with exercise acupuncture, and the patients in the osteopathy group were treated with osteopathy combined with routine rehabilitation. The changes of WOMAC score and ROM in three groups were compared before treatment and 4 weeks after treatment.@*RESULTS@#The overall effect of osteopathy group was better than that of routine group (=3.151, =0.005). The scores of WOMAC before and after treatment:pain of 23.84±4.66, 11.98±2.66, stiffness of 10.44±1.71, 6.42±0.74, daily life of 66.49±11.85, 35.80±4.44 in the routine group; pain of 22.64±3.22, 8.90±2.19, stiffness of 11.82±2.57 , 6.03±1.06, daily life of 71.72±13.59, 32.94±4.73 in the exercise acupuncture group; pain of 22.38±3.68, 10.66±2.75, stiffness of 11.81±2.08, 5.63±1.69, daily life of 69.69±8.96, 28.84±5.76 in the osteopathy group. Compared with the other two groups after treatment, the improvement of pain score in the exercise acupuncture group was better than those in the other two groups. There were no significant differences in stiffness score among the three groups. The degree of difficulty in daily life in the osteopathy group was better than that in the other two groups. The total score of WOMAC in the exercise acupuncture group and the osteopathy group were better than that in the conventional group, but there was no significant difference between the two groups.@*CONCLUSIONS@#On the basis of routine rehabilitation treatment, exercise needling and osteopathy have positive significance for the improvement of joint pain and dysfunction of joint movement in patients with traumatic knee arthritis, with certain popularlized value in the treatment of traumatic knee arthritis.

ABSTRACT

Objective To study the effect of Qishenhuoxue granule on serum inflammatory response in children with pneumonia complicated with sepsis. Methods A total of 138 children with pneumonia complicated with sepsis who were admitted to our hospital from July 2014 to June 2016 were enrolled in this study. Children were randomly divided into observation group and control group. The patients in both groups were treated with conventional Western medicine symptomatic support. The observation group was treated with Qishenhuoxue granule. The curative effect was determined after treatment. Interleukin-6 (IL-6), interleukin-10 (IL-10), tumour necrosis factor-α (TNF-α), serum procalcitonin (PCT), hypersensitive C-reactive protein (hs-CRP), CD4+CD25+regulatory T cell (Treg cells) ratio value, creatine kinase-MB (CK-MB), creatinine (Scr), aspartic transaminase (AST) and brain natriuretic peptide (BNP) were measured before and 7 days after the treatment. Data of mortality and duration of pediatric intensive care unit (PICU) stay during treatment and adverse events were recorded during treatment. Results The total effective rate was 89.86% in the observation group, which was significantly higher than that of control group (71.01%, P<0.01). The levels of IL-6, IL-10, TNF-α, PCT, hs-CRP and Treg cell ratio were significantly lower after 7-day treatment in two groups than those before treatment (P<0.05), and levels of IL-6, IL-10, TNF-α, PCT, hs-CRP and Treg cell ratio were significantly lower in observation group than those in control group ( P<0.05). The CK-MB, Scr, AST and BNP were reduced after 7-day treatment in two groups, and those data were significantly lower in observation group than those of the control group (P<0.05). The 28-day mortality and the number of PICU days were significantly lower in observation group than those in control group (P<0.05). No adverse reactions were found in the both groups. Conclusion Qishenhuoxue granule can decrease the inflammatory response in children with pneumonia complicated with sepsis and improve the therapeutic effect. It is safe and effective and worthy of clinical recommendation.

Objective To establish a rat model of chronic renal failure (CRF) by using 5/6 subtotal nephrectomy method , then observe and evaluate the effect of Turbidity‐reducing Granule on the intestinal immune function of rats with CRF .Methods CRF model Wistar rats were divided into high ,medium and low dose group ,sham operation group ,and model control group .respec‐tively given Turbidity‐reducing Granule 4 .05 ,8 .10 ,16 .20 g · kg -1 · d-1 .Benazepril group was given 2 mL/d (1 .2 mg/d) Benaze‐pril suspension fed;model control group and the sham group are saline 2 mL/d orally .The rats were observed before and after treat‐ment of signs and general condition ,serum BUN ,Scr changes ,and pathological changes were observed under microscope .ELISA method were used to detect serum and intestinal SIgA .Results Turbidity‐reducing Granule reduced the level of serum BUN ,Scr in rats with CRF ,there were statistically significant differences of Benazepril group and high ,medium ,low dose of Turbidity‐reducing Granule group compared with model group(P<0 .05) .Compared with the model group ,the intestinal SIgA of Benazepril group and low ,middle ,high dose of group increased significantly(P<0 .05);intestinal SIgA in ,middle and high dose group were significantly increased compared with Benazepril group(P<0 .05) .Conclusion Turbidity‐reducing Granule could improve renal function and in‐testinal immune function .

ABSTRACT

Objective: To investigate the effect of medicated serum containing Chinese medicinal formulae Tiaogan Sanjie (TGSJ) on proliferation of breast cancer MCF-7 cells in vitro , and to explore its possible mechanism. Methods: The medicated serum was prepared by using healthy male SD rats receiving TGSJ. The MCF-7 cells were divided into 3 groups: blank control group (cultured with normal nutrient fluid), negative control group (cultured with serum without medication) and TGSJ group (cultured with the medicated serum). After culture for 72 h, the cell viability was determined by MTT method, the expression level of p16 mRNA was detected by using semiquantitative RT-PCR, and the DNA methylation level of p 16 gene was detected by fluorescent quantitative MethyLight-PCR. Results: After culture with the medicated serum for 72 h, the proliferation rate of MCF-7 cells was obviously declined, the mRNA expression level of p 16 gene was obviously increased, and the methylation level of p 16 gene was obviously declined, as compared with those of the blank control group and the negative control group (both P < 0.05). Conclusion: TGSJ can inhibit the proliferation of breast cancer cells, and this effect may be related to reversing the methylation of p 16 gene and up-regulating the expression level of p16 mRNA.

ABSTRACT

Liver fibrosis is regarded as a form or status of liver regeneration disorder. Therefore, from a new perspective of regulating liver regeneration, we put forward a new strategy for the prevention and treatment of liver fibrosis. This strategy not only focuses on confrontational reverse regulation, but also emphasizes positive induction. Moreover, this strategy applies an integrated method of promotion and inhibition, which improves liver injury and regeneration and corrects the abnormal liver regeneration and the normal liver regeneration disorder. Furthermore, this strategy also advocates the combination of prevention and treatment, which prevents the occurrence of liver fibrosis or a disorder of liver regeneration and treats the rapid progression of liver fibrosis. The imbalance between epithelial-mesenchymal transition (EMT) and mesenchymal-epithelial transition (MET) is involved in the CCl4-induced liver fibrosis in rats. Diwu Yanggan capsules can delay the progression of liver fibrosis and attenuate the degree of fibrosis, which is associated with the regulation of liver regeneration by modulating the imbalance between EMT and MET (inhibiting EMT and/or promoting MET).

ObjectiveTo systematically evaluate the efficacy of traditional Chinese medicine/herbal decoction combined with ursodeoxycholic acid (UDCA) in the treatment of primary biliary cirrhosis (PBC), and to provide a reference for clinical medication. MethodsLiterature published before July 31, 2014 was searched in databases as follows: Cochrane Library, PubMed, China National Knowledge Infrastructure (CNKI), Chinese Scientific Journals Full-text Database (VIP), Chinese Biomedical Literature Database (CBM), and Wanfang Data. The randomized controlled trials (RCTs) comparing the efficacy of traditional Chinese medicine/herbal decoction combined with UDCA versus UDCA alone in PBC patients were included in the analysis. The methodological quality of included trials was assessed and the data were extracted, followed by meta-analysis using RevMan 5.0 software. ResultsA total of 12 RCTs were included, involving 681 patients with 346 in the test group and 335 in the control group. The results of meta-analysis showed that, after 6 months of treatment, the overall response rate, improvement rate, and biochemical indices of liver function (ALT, ALP, GGT, and TBil) and hepatic fibrosis in the test group were significantly improved compared with those in the control group (all P<0.05). There were no significant differences between the two groups in the immunological indices such as IgA, IgG, anti-mitochondrial antibody (AMA), and AMA M2 subtype (all P>0.05). ConclusionTraditional Chinese medicine/herbal decoction combined with UDCA markedly improves the indices of hepatocellular necrosis and cholestasis, degree of hepatic fibrosis, and clinical symptoms in PBC patients after 6 months of treatment, but leads to no significant improvement in immunological indices. Due to the limited number of included RCTs and patients through systematic evaluation, and the presence of selection bias and publication bias, more double-blind randomized controlled trials with large sample size, multicenter involvement, and high quality are required to provide convincing evidence.

ABSTRACT

This paper reviews traditional Chinese medicine (TCM) physicians′understanding of liver failure including its TCM causes, mechanisms, positions, and syndrome differentiation in various dynasties. The results suggest that modern researchers agree with ancient physicians on these aspects of liver failure. Based on achievements of ancient TCM physicians, modern researchers have further developed and improved their understanding of TCM causes, mechanisms, positions, and syndrome differentiation of liver failure. Moreover, this paper discusses the treatment of chronic liver failure with yang-supporting therapy, which provides a novel perspective and method for treating chronic liver failure.

Traditional Chinese medicine (TCM) treatment of liver diseases is derived from the regulation of liver function including storing blood and governing the free flow of qi, in which functional systems such as modern digestion, endocrine, and the gut-liver axis are involved, and is established on modern hepatic physiology, pathology, and etiology. To objectively reveal the characteristics and advantages of modern TCM treatment of liver diseases, we analyzed the clinical and research situation of TCM therapy for liver diseases in the last decade and collected major achievements that have been applied in clinical treatment of diseases, published in core journals, and confirmed by major scientific research programs. The results showed TCM combined with antiviral therapy can improve the clinical outcomes of chronic hepatitis B. TCM can help HBV carriers prevent disease progression. Integrated traditional Chinese and Western medicine therapy for acute-on-chronic liver failure can block the deterioration induced by endotoxin. TCM has been widely applied in protecting the liver through nonspecific anti-inflammation, alleviating hepatic fibrosis, and preventing non-alcoholic fatty liver. TCM plays an important role in treating some currently untreatable liver diseases. Therefore, it is our common responsibility to inherit and develop effective principle-method-recipe-medicines and create a better medical care system.

ObjectiveTo explore the clinical effect of oral administration and external application of traditional Chinese medicine on early stage of osteofascial compartment syndrome. Methods56 early osteofascial compartment syndrome patients had assigned in two groups randomly, each group had 28 patients. The treatment group used topo-application Xiaoyu tincture,after aseptic gauze sodden by physic liquor,affected limb would coverd by the aseptic gauze and change of dressing every two hours,oral administration of Xingqihuayu decoction. The control group used 250ml of 20% mannitol with dehydration therapy,two times everyday;654-2 bullet 10ml to intramuscular injection,three times every day. ResultsAfter the treatment,the results showed that the treatment group was much better than the control group at the time of ache abatement,swelling regression and disappear of all levels injury, arteriae dorsalis pedis changing. The difference had statistical significance( P < 0.05). ConclusionTopo-application chinese drugs pharmaceutics of Xiaoyu tincture and oral administration of Xingqihuayu decoction was a available nonspecific treatment to early osteofascial compartment syndrome.

ABSTRACT

Objective To explore the molecule mechanisms of ping-gan qianyang therapy on migraine rat model with the hyperactivity of liver-yang.Methods An animal model of migraine with hyperactivity of liver-yang was established through electrical trigeminal ganglion stimulation and administering orally with Fuzi decoction.The total proteins of adrenal#and in the rats were separated by immobilized PH gradient-based two-dimensional gel electrophoresis, and the 2-DE images Was analyzed by PDQuest 7.0 software.Matrix-Assisted Laser Desorption/Ionization-Time Of Flight Mass Spectrometry(MALDI-TOF-MS)and SWISS-PORT and MSDB database were used to identify differential proteins.Result The well-resolution and reproducible 2-DE patterns of rat adrenal glands in normal group,model group and therapy group were obtained.A total of 8 differentially expressed proteins were identified.Conclusion Ping-gan qianyang therapy may effect on migraine by regulating the expression of proteins related to energy metabolism and ubiquitin.

Objective To determine the effect of the Buyang Huanwu Tang and batroxobin treating sudden deafness.Methods 90 cues(99 ears)with sudden deafness were randomly divided into two groups,the control group were treated with dextran glucose intravenous and lignstrazine,the treat group were treated with the Buyang Huanwu Tang and batroxobin.The periods were 2 weeks.Results The effect in the treat group was more remarkable compared with the control group,there was a significant difference(P<0.05).Conclusion The Buyang Huanwu Tang and batroxobin treeing sudden deafness Can increase curative rete.

ABSTRACT

To investigate the efficacy of the formula composed of the herbal drugs to soothe the liver, regulate qi, clarify the heart and eliminate vexation plus acupuncture for general anxiety disorder. Methods: Forty cases in the treatment group were treated with acupuncture plus herbal formula and 38 cases in the control group were treated with oral Doxepin. The assessment was conducted by Self-rating Anxiety Scale (SAS) and the Treatment Emergent Symptom Scale (TESS) before, during and after treatment. Results: The total effective rate was respectively 82.50% in the treatment group and 84.21% in the control group, with a significant difference in SAS assessment (P<0.01) between the two groups before and after the treatment.There were no significant differences in the curative and remarkable effective rate, total effective rate and SAS assessment between the two groups (P>0.05), but the side effect was higher in the control group than in the treatment group (P<0.01). Conclusion: Acupuncture plus herbal formula can have a precise effective effect for extensive anxiety neurosis with mild toxic side effects.

ABSTRACT

Objective: To investigate the clinical efficacy of treating insomnia with mind-tranquilizing acupuncture. Method: Sixty-three patients were randomly divided into a treatment group of 32 cases and a control group of 31 cases. The treatment group received mind-tranquilizing acupuncture and the control group, oral depression-relieving and mind-calming herbal granules, for 30 days. Results: The response rate was 87.5% in the treatment group and 77.4% in the control group. There were significant differences in falling-sleep time, lasting period and times of nocturnal waking (P < 0.05) and a very significant difference in dream improvement (P< 0.01) between the two groups. There were no significant differences in afterwaking conditions and sleep evaluation (P> 0.05). Conclusion: Clinically mind-tranquilizing acupuncture has a better effect on insomnia than depression-relieving and mind-calming herbal granules.
